OUTDOOR PUBLIC FIREWORKS DISPLAY

We hereby make application for a permit to conduct a PUBLIC FIREWORKS DISPLAY and agree to comply with the laws of Oklahoma pertaining to that as set forth in the state statutes and NFPA 1123, 1126 & 160, current edition, as adopted by the Oklahoma State Fire Marshal Commission.

NOTICE: Fireworks are allowed to be brought on campus the day of the show. Notify UCO's Risk Management & Safety Operations Department/Fire Marshal of intentions as soon as possible. Please call 405-974-2345 and inform the dispatcher that fireworks are on location. Provide the description of the vehicle and any information requested. Security will need to be posted, and the fireworks should never be left unattended.

The name appearing on this permit must be registered in the current year of the Oklahoma Tax Commission as a listed company licensed to do business within the State of Oklahoma. An invoice listing the pyrotechnic material used in the show must be furnished to the University of Central Oklahoma and the State Fire Marshal. If this documentation cannot be produced, the fireworks may be seized as illegal O.S. 2012, Title 68 § 1628 (b) & (c). The effects operator’s plan for the show must be in writing and submitted along with the permit application form. The permit site plan must follow the same site plan outlined in NFPA 1123 - current edition, NFPA 1126 - current edition NFPA 160 - current edition adopted by the Oklahoma State Fire Marshal’s Office. After approval, the effects operators must keep this permit and information on site of the show for inspection by the Risk Management & Safety Operations/Fire Marshal and/or the State Fire Marshal.
